My iPhone XR can receive but won’t deliver texts to anyone who doesn’t have an iPhone. I have “send as sms” turned on in my settings and have no idea what else to do.

Contact your wireless carrier. SMS are their responsibilty.

"Send as SMS" has no effect on messages you're sending to an Android phone. What that setting does is, if an iMessage fails, it will try to send it again as an SMS. So, by definition, it only affects messages to other Apple devices.
As noted above, contact your carrier.
