iPhone 12 mini camera blurry/internal gear noise ( warranty replacement phone has the same issue)

Bought a new 12 mini and got the apple mag safe case. A week in the rear camera started freaking out when trying to take a photo and I could feel and hear some internal gear noise from the camera area. Sounds like it is struggling to auto focus and the camera is spasming (best way to describe it). The photos look insanely blurry.

I called apple and they sent me a new phone. About a week later the same issue is happening. On the phone with support now. Anyone have this issue?

Have you mounted your phone to motorcycle/bike handlebars. Engineering came back and told the agent that the OIS is probably fried from vibration...waiting to see if they will replace this under warranty. Nowhere does apple say not to mount a phone to a bike.
