Unauthorised Use of Apple TV Account

I left my old Apple TV device in my old flat in Edinburgh at the beginning of this year and it has been found and is now being used by someone else to watch my movies and listen to my music without my permission.

Please advise on how I can disconnect this other unknown user from my account.
