If you purchased AppleCare+ and it is still valid, go to Settings > General > About

Just below Serial Number will be line which will tell you if you have a valid plan. If you don't, you don't.

Even with a valid AppleCare+ plan a replacement will not be free. It will just be greatly reduced from what is called an Out of Warranty replacement cost, which is about 60% of the cost of the phone when new.
