Numbers Spreadsheets Auto Duplicating on iPhone

I have just started attempting to use Numbers on my iPhone 6. Everytime I open a spreadsheet, I get an additional copy of it on the Numbers List - even if I just view it. I don't want an additional copy. I want one copy that I can edit and save.

Am I missing a "save" step? Or is there something else I need to do?

I need to edit this file during a meeting, so I need to do it on the iphone.

My issue is though, that sometimes I make a change and close the app and it did not save it. Every day I got some changes it does not save.
