Automatic updates never have worked

I have had the automatic update option turned on for iOS updates since it was released. I’ve had 3 separate apple devices since that feature came out and it has never worked ever. I have my phone set to automatically download and install iOS updates, the phone is basically always connected to WiFi, and is almost always charging. This is especially true at night. Why has this feature never downloaded and installed a single update? I’m currently using an iPhone XR.
