Is there a way to create an Apple ID for a service account for a shared iPad? I can use an account email address but what about the birthday and phone number?

two Apple IDs on one telephone account? Can two different Apple IDs on two different iPhone share the same account with the phone company? The telephone bill is in the name of User 1. User 2 is now contemplating adding a second iPhone to the account. For professional reasons, both Users must maintain their own separate Apple IDs. Since you are creating the Apple ID, you can use your information so you remember it if the account has to be recovered.
