Are there significant differences between Appletv app and Apple TV device? I have Appletv app subscription without the Apple TV hardware. Is one better than the other?

The subscription is not for the app, but for the streaming service of Apple Original content: Apple TV+. It may be a bit confusing with all the similar names, but the Plus makes the subscription a different ‘product’, not better. The Apple TV+ Channel is just a part of what is offered in the Apple TV app. See here what Apple TV+ is all about: Apple TV+ - Apple.
