First check Activity Monitor and tell us which apps are using too much CPU. Post here a screenshot of Activity Monitor so can check it either.

I'd say you better get back to High Sierra, it's a more modern and very reliable version.

Etrecheck is saying your SSD is running to slow, so try replacing the flat cable to see if helps. Flat cables are often a problem in 2011 and 2012 Macs.

You can try installing High Sierra on an external USB drive to see if it runs slow too. If it runs better, problem probably lies on the SSD or flat cable.

Create a bootable USB installer so you have a local copy of macOS and will not depend of the internet for the process.

Unless you have removed CleanMyMac (CMM) with extreme prejudice, you are not done. We have seen EtreCheck reports here where CMM reduced disk performance significantly, enough to trigger Etrecheck's "failing drive" flag.. It is also the likely cause of the high CPU usage at the time of your first test.

Did you do a second test to see if the SSD performance is back up to nominals? Nominals for a 6GB/sec in a 2011 MBP are twice what your first report shows. My 2012 MBP has the same drive bus specs as yours but post this with an inexpensive OWC SSD:

Performance:

    System Load: 1.61 (1 min ago) 1.60 (5 min ago) 2.94 (15 min ago)

    Nominal I/O speed: 0.15 MB/s

    File system: 28.20 seconds

    Write speed: 487 MB/s

    Read speed: 482 MB/s

Twice yours. All 2011 and later Macs have a 6GB/sec SATA III drive bus so the drop in SSD performance has to be the SSD or the cable.

Please test again to see if your SSD speeds are now closer to mine.
