Trade phone Plan

Hello, I am currently enrolled in the monthly payment plan with my phone with the option of upgrading the phone after reaching certain payments. I am elegible now to change my phone. How can I start the process to upgrade my phone?

iPhone Trade-Up program - https://www.apple.com/iphone/trade-up/ - "Just trade in your eligible smartphone, and then use the credit to lower the full cost of a new iPhone or to reduce the monthly payments with your carrier." Not available in all countries
