voicemail option no longer within phone icon choices

Have a iphone 11. ios 14.3 Was working just fine and all of a sudden I no longer have the voicemail selection option within the phone icon (favorites, search, keypad, etc - only have 4, not 5). Cannot find how I access VM. VM is working when someone calls to leave a message. I just can't retrieve and am not notiifed there is a vm. Other family member has exact same phone / ios on the same carrier bill / service. He has no problems and the VM is an option within his phone icon. How do I get this back and functioning properly.
