how do I unlock my daughters disabled iphone

My daughter changed her password and then forgot it, for the iphone and for apple id. I have reset the apple id and need help getting her phone to unlock. All is says is that it is disabled and to connect to itunes. Can someone give me more direction on how to fix this?
