How do I create a fillable form in Pages to export as PDF???

Because Pages is no Word clone, it does not provide a form designer or any means to create fillable forms.

Use MS Word, or the free LibreOffice Suite, as both have built-in Forms design tools. Chapter 18 of the LibreOffice Writer User Guide is devoted to form creation.
