When saving to desktop (or renaming Preview files) why do new file icons not appear? Why do new Preview names do not update old ones?
Machine is MacBook Pro 2016 15" using MacOS 10.13.6 (High Sierra).

Since the issue doesn't occur in the new user account, this indicates the issue is specific to your user account.
From here, try removing the login items on your user account in System Preferences > Users & Groups, restart your Mac, and test for the issue again.
If the issue is resolved with all login items removed, you can start adding them back individually to try to isolate the cause of the saving issue further.

Not every file. It seems only files saved to the Desktop. If in the Save dialog I create a new folder, the new folder does not appear.
More accurately, the saved or created items eventually appear, typically a minute or so later. I can force them to appear by relaunching the Finder.
When I try to Rename a Screen Shot, the new name does not immediately appear. But it will appear a minute or so later, and can be forced to appear by relaunching the Finder.
As per your advice, I tried Safe Mode. The same problem persists in Safe Mode.
I've got another account that I've used as a test. The problem does not persist when I boot up into that account.

In Users and Groups there are no login items listed. So the problem is evidently independent of such things.
I should perhaps add that this has been an intermittent problem for several months. Over some protracted periods, it would go away, seemingly as a result of zapping the PRAM. But this time, I've tried zapping PRAM several times and the problem persists.
I've tried to think of a possible correlation with other things going in or out of my machine. To my knowledge, I've done nothing different, and have made no changes unlike activity that took place prior to the problem's appearance. It's a real head-scratcher.
