Internet Recovery / Apple ID / etc... nothing works!

Trying to reinstall on MacBook (2015) using Internet Recovery. After submitting my Apple ID, password, my phone receives message that my Apple ID is being used on a device located.... blah blah. I submit allow (because it's me). A passcode pops up on my phone, but the computer does not have the boxes to input the passcode. Thus I can't get past this part to complete the reinstall. Thx in advance!

If you see a password box, enter the password (ABC) followed by the code (123) or ABC123 (no spaces).
