Where can I find my PUK code?

Use a SIM PIN [PUKcode] for your iPhone or iPad --> https://support.apple.com/en-us/HT201529

Ask your carrier to help you unlock your SIM card using the default SIM PIN or PUK code.

Your carrier should have your SIM PIN & PUK codes.
