No app is allowed to use my camera, but occasionally , a "camera used recently" text appear when I unlock my device, Iphone 11 pro with software version 14.4.
I have Instagram, Messenger, and whatsapp installed, but again, they don't have permissions to use my camera.

Weird. Have you tried truning off Screen Time completely in Settings?
Screen Time has such a complex set of prevention rules that since 14.x it is a good place to start.
It can prevent you allowing other things so you may still need to allow apps to use camera.

it just happened 1 minute ago, The only app “opened” was the in-built “Messages”
any ideas are welcomed.
