Very slow, beachballing Mac

My Mac's been running very slowly for more than a year. Opening any app takes minutes. Genius Bar erased hard drive last fall, and I restored from TimeMachine. No appreciable change. Etrecheck suggested deleting AdobeFlash. Done, but still no change in speed. Here's the report. Thanks in advance for any help you can offer.

Your report shows 2 things I noticed. First it reports the SSD is operating slowly, it also shows that you may have adware installed. Please run Malwarebytes and then see if that helps the performance. If it does not then I suspect you Fusion drive may be failing which would mean the computer should be taken to your local AASP to be serviced.

I would also consider upgrade the RAM to a minimum of 16GB. If you decide to do this, please only use Crucial.com or Macsales.com RAM. iMacs are sensitive to quality RAM and both vendors supply high quality products at very reasonable prices and have excellent service.

I'd replace the drive if that is the problem. An AASP can thoroughly test it, you may want to get rid of the Fusion drive all together and install a SSD. If it is the drive, I'd install an external SSD (USB 3 drive like the example below) which would be much less expensive than doing an internal repair.

Thanks again! I could ask you questions until you get tired of me, but I should be more respectful of your time, so last one (maybe!): Is the paid version of Etrecheck worth $17 for a novice like me? Since Genius Bar didn't catch the Fusion Drive problem in the fall (they just erased the drive & told me to take it back home and restore from TimeMachine), maybe I could resolve other problems myself. Again, I'm pretty novice, but can follow instructions if they're simple & clear.
