Apple TV 4K Up Next skipping episodes or movies

In the past couple of months, I've noticed the Up Next feature (via Apple TV 4K) has a bug. If I have saved an episode of a series that's next, it will skip to the next episode. If I have a movie in Up Next, it shows as if I've watched it almost to the end, even if I have just started it. I put content in Up Next either by searching it and choosing Add to Up Next, or by starting the episode/movie, pausing it, then exiting to the Apple TV Home Screen. At that time, it's saved properly, but the next time I look, it has changed as I described. I don't think this has happened with Apple TV+ content; it only seems to happen with other apps such as Hulu, HBO Max, Prime, etc. I don't know if it's a bug in the Up Next feature itself, or miscommunication between Apple TV and the channel apps. Has anyone else experienced this, and do you know how to fix it?
