How do I set up separate medical profiles for me and my wife whit our iPhones linked on the cloud? Whenever I set up one it shows up on the other phone

Immediately stop sharing an Apple ID. Each person, regardless of age, needs her/his own, unique Apple ID.
The person acquiring a new Apple ID must thoroughly cleanse her/his device(s) as follows:

Then that person “moves in” to the like-new (software-wise) device(s).
You may set up Family Sharing if you wish.
