Apple care theft and lost

I have lost my iPhone and agreed with the terms to set lost mode. However, I didn’t receive a claim number nor get an replacement. What to do or how to apply for a replacement?

Apple Care+:- file a claim for your lost or stolen iPhone. (If you had, Find My enabled)
