My child has purchased an item with my account, what can I do to appeal and get a refund for that amount. I want to chat directly with the support person!

Purchases are obviously considered final as you know, but you could try the 'report a problem' page to contact iTunes Support and see if they will refund you as a one-off : http://reportaproblem.apple.com
