Contacting Apple Support:
Seems to be a glitch on apple's database -- Something Apple needs to investigate. So, having read everything you've mentioned, I would contact Apple Support, and have them modify it to what need be on their end.
Screenshare with Apple:
Ask the Apple Rep to perform a screensharing session with then. What you would do is enable the speaker phone on your iPhone, and explain to them what is occurring. Pretty much your would hold the iPhone in your hands, while talking with the Apple Rep. Show them where you are seeing the issue. Screensharing will allow the Appel Rep to see this from your perspective.
