What does the Apple watch hard fall alert do if you fall in a foreign country? Does it automatically know what emergency service to contact?

Short version: yes it does.

"Some countries and regions have multiple emergency service numbers. For these countries, Apple Watch will call the number associated with ambulance services."
And this does apply to traveling in foreign countries.
