You absolutely need a working remote to setup the Apple TV app for 3rd gen Apple TVs or earlier.
Your only option is to borrow one from a friend or relative just to navigate to the remote settings on the Apple TV and set up the App on the iPhone.
I apologize for my obtuseness, but how do you follow this instruction if you don't have the original ATV remote (since it envisions a scenario in which you've lost that device?
