Your AirPort Extreme is "remembering" the settings that it used to connect to your "old" modem. It does not know that you have a new modem, so it is lost and does not know how to find its way.
You will need to help your AirPort Extreme "forget" the settings that it used to connect to your old modem, so that it can be reconfigured again to receive the new settings that it needs from your new modem to work correctly.
Perform a Hard Reset by holding in the reset button on the AirPort Extreme for 7-8 seconds to erase the "old" network settings and then set up the AirPort Extreme again, just like you did originally.
Connect the Ethernet cable from the modem to the "O" port on the AirPort Extreme.
Then, click the WiFi menu and click directly on AirPort Extreme under the listing of New AirPort Base Station and the AirPort Utility setup utility will open up and guide you through.
You can use the same names and passwords if you wish, or use new names and passwords.
