Are you using Photos for Macor Photos on the iPhone? If yes, which system version? In Photos 6 on Big Sur try this:

How to extract a Still Frame from a Video Clip in Photos 6 on Big Sur… - Apple Community


In earlier versions of Photos you could save a still frame from the actions menu when playing the Video.

  1. In the Photos app  on your Mac, double-click the video clip to open it.
  2. Position the pointer over the clip to display the player controls, then click the Action pop-up menu  and choose "Save Still Frame".



On the iPhone, play the video and save a screenshot to Photos.
