Mountain Lion was the last in this cycle. I remember OS 6 and most of OS7 were free too. It was the OS7.6 - OSX 10.8 that you had to pay for as upgrades (all Macs come with their original OS as part of the package).

Am I right in thinking that Snow Leopard was the last Mac OS X which was distributed on a boxed disk?

I still have my Snow Leopard Server edition, still wrapped. I think I got almost the last copy Apple had. It took somebody at Apple HQ 10 minutes to find a copy. I know that was a highly desirable version because of its potential for being run as a virtual system.
Yes, though Lion had a limitec release on a thumb drive. An upgrade was later released for SL which had the Mac App Store app for upgrade to online versions.
