When I try to activate call forwarding on my iPhone 5s it turns itself off immediately.
I go to call forwarding. toggle it to on, then press forward to I add my number press + go back the button then toggles to off and my number has been erased. Any suggestions please.

Not all carriers support controlling that from the settings on the phone. If yours does and it's not working, they may have to re-provision your account.
