How do I setup all email accounts to backup to iCloud drive in iCloud backup configuration? I know I get iCloud mail account as a default and that backs up but not the other email accounts using 'Other' setup.

You cannot. iCloud backups do not backup any email. Emails are held on the server of the email service provider, so backing them up would be a needless use of iCloud storage space.
