Trying to scan a document using my phone. Instructions state pressing certain buttons that I don’t have while in notes. My phone has been updated. Don’t know what I am missing

You aren’t the 1st to find this troublingly non-intuitive.
It’s in the “Notes” app.
Start a new note, then click the “camera” icon within the note editing window. (NOT the actual “Camera” app)
(“starting” a new note is THE step often overlooked)
Select scan document.
It will create a note which can be “sent” as a pdf.
Once you figure it out, it actually works reasonably well for digitizing documents to pdf “on the cheap.”

Also, make sure the Note you start in the Notes app, is started under the "iCloud" or "On My iPhone" folders.
Notes started under 3rd party email provider account folders will not have the document scanning option.
And just so its clear, I'll repeat it, Scanning is done in the Notes app, not the Photos app.
