Voicemail Constantly Resets

How do I get my iPhone to stop making me set up voicemail over and over? It makes me set it up as new (usually after a reboot or phone restart) despite having done it numerous times. This is a relatively recent issue so possibly something to do with an iOS update. It’s extremely frustrating as I don’t even know it needs to be set up again unless I tap the voicemail tab. I don’t even receive notifications of voicemails when this happens. Help me Apple!

I don’t think it’s carrier related. It seems pretty specific to the voicemail tab on the phone app. The actual voicemail (in the cloud or wherever the carrier stores it) still works, I just don’t know it because my phone says “To retrieve a voicemail first set a password and greeting.” So once I complete that (for the umpteenth time) any voicemails I’ve received will populate in the voicemail tab and appear as normal.
