$200 Rebate for Connecting iPad Pro generation 5 to Verizon Line

I have established a new line for my new iPad Pro with Verizon and cell service works fine. I have looked and looked on Verizon's site and cannot figure out how to get the $200 rebate that is advertised by Apple. Has anyone done this with Verizon? If so, where do I find the application on Verizon's site?
