That’s your line identifier since you have two lines.
The phone defaults the line names to “Primary” and “Secondary” during initial setup; and the flag just shows the 1st letter.
If you renamed your lines (e.g. to “T-Mobile” and “Verizon”) it would display either a T or a V.

But to get that signal indication would there not need to ALSO be Hard SIM in addition to the eSIM ?… 🤔

Is your primary AT&T line an eSIM … or a Hard-SIM?
Is there any hard SIM physically installed?
You can also check in Settings -> Mobile[Cellular] -> Mobile[Cellular] Plans.
If there are indeed two plans “showing” you can simply turn “off” the one you’re not using.
When you go to dial, are you presented with options to select a line:
(you have to “click” to get the dropdown shown below)
So the only way I would have that is if I had two lines, because I don't. Could my iWatch line cause that to happen? Or maybe a FaceTime ID?
No, you don't have to have two lines you merely have to have a dual line phone. You have activated one, the Primary, and that is the one you are using.

His signal strength is showing two “lines” (even though only one has a signal)
When the “2nd line” is “off” … it doesn’t matter which one … the signal strength meter becomes “normal” again.
They may have activated an eSIM and it was done incorrectly. But they would need to contact AT&T to fix that issue.
