iPhone unlock icon and passcode screen- HACKED?

The screenshot below would appear on my iPhone multiple times. The only way to get rid of it was to roofer off my iPhone and then turn it back on. I can’t understand how it why the iPhone would give me an unlocked padlock icon but request a passcode at the same time. I already showed them this at the Apple store. Of course they had no answers. This snafu was with my iPhone10. Is my phone hacked?
