I've read that calls will not be silenced if Do Not Disturb is turned off, regardless of the setting of Silence. With the setting as below I keep missing calls I should be able to answer.
Phone: iPhone 8
iOS: 14.6
Settings:
Do Not Disturb: Off
Scheduled: Off
Silence: When iPhone is Locked
Allow Calls From: Everyone
Repeated Calls: On
Silent Mode: Off
Auto-Lock: Off
What am I missing?

I'll assume you mean Silence Unknown Callers = Off. That setting needed to be changed from On to Off.
