How do I find out what this app is in my menu bar?
I don't see anything that looks like it in my applications folder. If I find out it's something I should eliminate, how do I do that?

Take a look in your System Preferences and your Applications folder (or, if you use it, the Launchpad) - usually, the apps themselves are not in the menu bar, but in the dock or in the Applications folder. But they may "park" some options, etc. in the menu bar.

It is Canon hdAlbum EZ, and "Allow Notifications" is turned off.

I've looked in those places (and in the other applications folder). I don't see anything that looks like that icon.
Try looking in Apple Menu > System Preferences > Notifications - scroll down the pane on the left and see if you see anything that looks like it.
