Reused passwords/Compromised passwords

Keychain is telling me 50 of my passwords are appearing as 'reused', when they are not, each one is unique. Furthermore, the rest of my passwords (all 96) are all listed as appearing in data leaks. I have already change all my passwords every year, but this keeps occurring, even when I respond to advisories to change them. Changing them makes no difference, I get the same 'reused' or 'data leak' advisory.


Some guidance/advice, please.
