Find my iPhone

im wondering if find my iPhone can be totally wrong on someone’s location, or if it possibly means someone was hacked? I checked someone’s location and it said their current location was almost 400 miles away from where they actually were. I refreshed it many times and it still gave the incorrect location. About 20 minutes after, it changed and said the correct location. Which was almost 400 miles away…. Now I’m concerned this persons Apple ID has been hacked?

See the section titled, "What if I don’t recognize the location shown in my sign in notification?" by clicking on the link for this support article 'Two-factor authentication for Apple ID' --> https://support.apple.com/HT204915

Yes, depending upon the resources it is using for finding a location it can be wrong. If it is having to use the IP address of a wi-fi station to get a location, it is possible that the location reported is the headquarters of the Internet service providing the wi-fi connection.
