Hello I have iPhone 11 pro max and the adapter gets really hot nowadays whenever I am charging my mobile phone, what could be the matter ?

It is normal to get warm. However, if it gets too hot to touch, it should not be used.
1) Start here:
Identify counterfeit or uncertified Lightning connector accessories
2) Check the charging port for lint or other debris. And see this:
