windows icloud passwords "approve" button does nothing

I'm trying to setup the new iCloud passwords option on Windows 10.

I have followed all of the instructions that I've found online.

I have 2FA setup on my iPhone.

I have the iCloud passwords extension active on Chrome.

On the iCloud app on windows, the "passwords" check box is greyed out, but "approve" button is not. When I click the "approve" button I get asked for apple id/password, then nothing happens and the iCloud app display is the same. No 2FA is asked for on my iPhone either.

The Edge / Chrome "install extensions" buttons are also grayed out.
