I am looking into devices and options for an elderly lady. I would like fall detection and possibly other minor health monitoring like heart rate and such. Can an apple watch with only GPS and one of the fall
detection apps be used and work similar to an apple watch with cellular service?

Fall detection does not require a GPS+Celluar watch. However, the GPS only watch will need to be near the paired iPhone for fall detraction to work. For more info, see:

Fall detection requires an Apple Watch Series 4 or later.
A GPS only watch can do everything a GPS+Cellular watch can do as long as it is near the paired iPhone.
