What would cause iPhone 12 pro max not t ring or alarm to sound at certain times like do not disturb is on, when do not disturb is turned off? I've checked all the sound settings and even had people test call and it will ring but not in the mornings.

Which exact version of iOS are you updated to under Settings > General > About? Does restarting resolve the issue? Are the callers in your Contacts app? If not, check to see if Silence Unknown Callers is turned on as described here: Detect and block spam phone calls
