iPhone 12 - voice breaking up on calls

I have an iPhone 12 Pro. Every time I’m on the phone to someone it says I’m breaking up.

noise cancellation is turned off, microphone isn’t covered. I have signal or wifi so I’m not sure why.

any suggestions?
