In addition, check: Settings>ScreenTime>Content & Privacy Restrictions>Itunes & AppStore Purchases... the first 3 options must be set to "Allow".

If its been paid for it should have started the download at that time.
What happens if you go back to the App Store and try to download it?
Does it appear in your purchase history? If so, can you download it from there?
