What support does Apple offer to parents of special needs children over the age of 18 who dont understand what they are purchasing and need controls put on their purchasing ability via iphones.

You can restrict and remove access to many things on the device via the Screen Time controls on the device. You may want to consider restricting the use of in-app purchases and their ability to download apps on the device to prevent this behavior. This will remove the App Store and iTunes Stores from the devices Home Screen and can only be accessed by a passcode you set as a parent / guardian. See: https://support.apple.com/en-us/HT204396 and https://support.apple.com/en-us/HT208982 as well as https://support.apple.com/en-us/HT201304. Those should give you a good idea where to head.
