How does Screen time work on Mac?

Does Screen Time record the usage of all the currently open apps or only the one whose name is in the menu bar?

Screen time widget showing the time from the clock I do not really understand it but my mac has been showing the usage time of the whole day. Does it have somethng to do with some program running in the background or something, I do not know. Here's an image. If you happen to know the solution I would be happy to hear about it.
