With the updated information it sounds as though you’re seeing the grayed out squares when access the app preview screen that displays opened apps as outlined in the following article.
Are you noticing the squares appear where there was an app preview showing previously? If so, does the gray square appear if you return to the app preview screen?
You mentioned that your iPad is fully updated. To confirm, you’ve updated to iPadOS 15.0.2?
Update your iPhone, iPad, or iPod touch
If so, the next step would be to contact Apple. You can find their contact details online via the Get Support website.

To clarify, where are you seeing these appear? For example, are you using a specific application or do they appear elsewhere? What steps do you take immediately prior to these appearing?

It’s my iPad Pro 12.9”. These grey squares happen every time I minimise an app. They only last for 1 or 2 seconds then disappear. I have re booted my iPad but no change. This is a new iPad and my previous 11” version never displayed this anomaly. BTW the SW is fully up to date. This is not the Accessibility magnifier either 😊
