What do I do when my phone has been hacked?

My phone has clearly been hacked into. My mac seems to have also been hacked into. Does anyone know how to fix this without having to completely wipe everything back to factory settings? Also, if a hacker has gotten into my wifi do you know how I can fix that too? Thank you!!
