What if I didn't receive either a QR code or a downloadable file?

For both shots, the nurse gave me a filled out 3" x 4" CDC COVID-19 Vaccination Record Card. My card includes a Patient #. Do I have to go back to my Primarcy Care Physician and use this # to request a downloadable file, log into my county's health department website, or do something else? The Apple documentation should be clearer.

We understand that you're attempting to add your COVID-19 Vaccination Record to your Wallet but you didn't receive a downloadable file or a QR code. We see that you've also reviewed Add verifiable COVID-19 vaccination information to Apple Wallet and Health.


In order to add your COVID-19 Vaccination Record to your Wallet, you will need either of these 2 items, the QR code or the downloadable file. You won't have the option to add it if you don't have either.
